Open Access Colonoscopy Screening Contact Form
- This form can be printed, filled out and faxed or brought in to our office when requesting a screening colonoscopy. After we receive this information, someone from our office will contact you to obtain more information and to set up the procedure.

- Please print and fill out as much as you can prior to your appointment. Upon arrival, the nursing staff will help you with whatever areas you don't understand or for which don't have the information. Accuracy in filling out the actual medications and dosages is of utmost importance. This list should include any over the counter medicines, vitamins or other dietary supplements.
- If you are confused about the medication names, please bring in your medication bottles.
- Please also remember to bring up to date insurance information, cash, check or credit card to pay your co-pay or visit payment.
