We're Going Digital
East Bank Gastroenterology is going digital. On 20 June 2011 we went live with the PowerWorks ASP EHR. This makes it important for first time patients to show up at least 45 minutes prior to their scheduled appointment time. Once your information is in the electronic health record (EHR), the pay-off will be better access to your information and, generally quicker response times to medication refills, health questions, etc.

Patient Forms
For your convenience, our patient forms are located below. Please download, print and complete the following forms so we can expedite your first visit:
Open Access Colonoscopy Screening Contact Form
- This form can be printed, filled out and faxed or brought in to our office when requesting a screening colonoscopy. After we receive this information, someone from our office will contact you to obtain more information and to set up the procedure.
East Bank Gastroenterology New Patient Info Forms
- Please print and fill out as much as you can prior to your appointment. Upon arrival, the nursing staff will help you with whatever areas you don't understand or for which don't have the information. Accuracy in filling out the actual medications and dosages is of utmost importance. This list should include any over the counter medicines, vitamins or other dietary supplements.
- If you are confused about the medication names, please bring in your medication bottles.
- Please also remember to bring up to date insurance information, cash, check or credit card to pay your co-pay or visit payment.

Insurance and Billing
Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are happy to file insurance for your reimbursement as long as you are free to choose your own gastroenterologist.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Educate yourself on your symptoms by reviewing the content on this website. We are located 2 blocks from East Jefferson General Hospital, between the hospital and Veterans Highway.
Across Kawanee from East Jefferson Wellness Center, on Houma Boulevard.
LaPlace Office
We are located just outside the LaPlace city limits, near the Belle Terre I-10 exit, in the single story row of offices across Rue de Sante from River Parishes Hospital.
